Common Uses:
Carbimazole is used to treat hyperthyroidism. Carbimazole is a pro-drug as after absorption it is converted to the active form, methimazole. Methimazole prevents the peroxidase enzyme from coupling and iodinating the tyrosine residues on thyroglobulin, hence reducing the production of the thyroid hormones T3 and T4 (thyroxine).
Off Label Uses:
Currently there are no off label uses for carbimazole.
Side Effects:
Some of the common side effects of carbimazole include rashes and pruritus, which can always be treated with antihistamines without stopping the carbimazole. For those patients where sensitivity reactions can not be controlled, propylthiouracil may be used as an alternative.
Its most serious rare side effect is bone marrow suppression causing neutropenia and agranulocytosis. This may occur at any stage during treatment and without warning. Patients are advised to immediately report symptoms of infection, especially sore throats, so that a full blood count test may be arranged. If this confirms a low neutrophil count then the drug must be discontinued immediately, allowing for usually a prompt recovery. However failure to report suggestive symptoms or delays in considering the possibility of immunosuppression and its testing, can lead to fatalities. Should be used with caution in pregnancy as crosses the placenta barrier.
Cautions / Warnings:
Special precautions need to be exercised while administering carbimazole to people with the following conditions: People with liver conditions, pregnant women, and nursing mothers.
Drug Interactions:
Carbimazole may interact adversely with other medicines. Iodine or iodine excess may decrease the response to carbimazole, requiring an increase in dosage or longer duration of therapy with antithyroid agents.
As thyroid and metabolic status of patient decreases toward normal, response to oral anticoagulants may decrease, however, if thioamide-induced hypoprothrombinemia occurs, anticoagulant effect may be enhanced. Adjustment of oral anticoagulant dosage on the basis of prothrombin time is recommended. Serum concentrations of digoxin and digitoxin have been reported to increase as the thyroid and metabolic status of patients taking antithyroid agents decreased, reduction in dosage of any digitalis glycoside may be necessary as patients become euthyroid.
Antithyroid agents may decrease thyroidal uptake of sodium iodide I 131, a rebound increase in uptake may occur up to 5 days after sudden withdrawal of Carbimazole.

About Carbimazole
Carbimazole belongs to the class of sulfur-containing imidazole derivative agents and is used in the treatment and management of thyroid diseases. Carbimazole is an imidazole antithyroid medication belonging to the group of drugs which are referred to as pro-drugs. It is known as a pro-drug mainly because it converts to methimazole after being absorbed by the body, generating an antithyroid action that works against hyperthyroidism (excessive production of thyroid hormones) and thyrotoxicosis (inflammation of the thyroid gland). Carbimazole is sold under different brand names, which include Atirozidina, Basolest, CG1, Carbimazol Spofa, Carbotiroid, Mertiran, Neo-Mercazole, Neo-Theyreostat, Neo-Tireol, Neomercazole and Tyrazol, among others. It is generally indicated for hyperthyroidism, when preparing a patient for thyroidectomy (surgical removal of all or part of the thyroid gland), or used together with radio-iodine therapy in cases of hyperthyroidism.
